The test of an implementation is not the first audit but the third. By then the consultant has gone, two of the trained staff have moved on, and the system either runs on habit or has quietly reverted. What separates the two outcomes is almost always the procedures themselves: written from the real process, or from an idealised one nobody recognised in the first place.

The cost of documenting fiction

It is tempting to write down the process as it ought to run and hope practice catches up. It never does. What follows is a permanent gap between the manual and the floor, which staff resolve by ignoring the manual and management resolves by rehearsing before audits. Documenting the messy real process and improving it from there is slower to start and vastly cheaper to maintain.
